Please fill out the accident report:

• E-bike

• E-moped

1. Information about the opposing party (full name, insurance details, address, email and phone number).

2. Information about witnesses (full name, address, email and phone number).

3. Case number from police. 

4. Photos of: 

   1. Accident scene - from different perspectives and directions if possible.

   2. Dance vehicle: 

      1. From all four sides, making sure licence plate is visible

      2. Detailed view of the damages

   3. Opposing party vehicle:

      1. All four sides of the other vehicle, making sure licence plate is visible

      2. Detailed view of the other vehicle's damages
